How raid access
actually works.
Open-world raid bosses no longer spawn. Three different things are called “instances” and only one of them contains a raid boss. Getting that wrong costs you a charge and an evening.

What changed
In classic EverQuest you found a raid boss by going where it stood and waiting for it to be up. That is not how Legends works, and it is the single assumption most inherited advice still makes.
Open world
Raid bosses do not spawn in the open world. The 16 June patch names them individually: Maestro of Rancor, Innoruuk, the Prince of Hate, Cazic Thule, Fright, Terror, Dread and a dracoliche “no longer spawn in the open world & personal instances”. Nothing since has reversed it.
Voidlings
Raid instances are created by NPCs called voidlings. Hail one and it offers a list of raid options along with their lockout timers. That is the only route to a raid boss.
The three instance types
They share a word and almost nothing else.
Solo raid versions exist
This is documented rather than folklore, and it is the reason small-group raiding is worth planning around at all.
The 16 June patch note lists, verbatim: “Balance changes to Solo Raid versions of Innoruuk, Cazic-Thule, Maestro of Rancor, and Lord of Loathing.” A thing cannot be rebalanced unless it exists. Four named raid bosses have separately tuned solo versions.
Difficulty D0 to D4 is selectable when creating a raid instance, and difficulty does not raise mob levels — it changes what mobs do.
